Why doesn't my title or pay match what I actually do?
You run the practice. You know the patients better than the doctors do. You carry the memory, the relationships, the things that would fall apart without you. None of that is in question. What is in question: have you ever put this in front of the person who sets your pay, in terms they can act on? Feeling unseen is not the same as being told no. You haven't been told no. You haven't asked. You can keep waiting to be noticed. Noticing is not coming. Or you can choose to make your worth impossible to ignore — in writing, with a number attached. That is the only move left that is actually yours to make.
Your title will not catch up on its own. Titles and pay follow what gets written down and asked for — not what gets quietly carried. You have not failed to earn recognition; you have failed to demand it. Name the work in writing, attach a number, and ask. That is the only path that exists.
What changes unlock by starting
- A written record of your actual work, hard to dismiss — even by you.
- A real number to anchor the conversation, not just a feeling of being owed.
- A scheduled conversation that finally happens, instead of a resentment that never gets said.
- A clear answer — yes or no — about whether this practice will ever pay for what you give it.
